Responsible for assembling and maintaining patients' health information in medical records and charts. Ensures all forms are completed and properly identified and signed and all necessary information is in the computer. Communicates with physicians or others to clarify diagnoses or get additional information. May also assign a code to each diagnosis and procedure. Under FLSA, incumbents in this position are nonexempt.
